To be considered for academic or non-academic accommodations, students should submit appropriate documentation to support their request. Documentation often helps to fill in gaps that are not evident in the student's self report or subsequent discussions. The packets found below are an easy guide for professionals to provide this information to the Office of Academic Support & Accessibility. If a provider prefers to submit a letter, the letter should contain the same information requested in the appropriate packet. Generally, the documentation should be uploaded at the time the student submits his or her request for accommodations. Lyon College welcomes suggestions for accommodations from the provider, but ultimately the college is responsible for determining reasonable accommodations as instructed by ADAA, Section 504, and other applicable disability law.
